Point tenderness is a clinical finding that refers to a patient's ability to clearly identify (point to) a specific pain location. In contrast, diffuse pain is generalized to a broader body region. When an orthopedic injury is involved, point tenderness suggests fracture, whereas diffuse pain suggests muscle or ligamentous damage or chronic injury (degeneration, inflammation, etc.).
